Several members were required to pay a late filing fee when they submitted their 2012 City of Chicago Ethics Statement of Interests. The Lodge attempted to resolve this issue by appearing at the Board of Ethics to appeal its decision. The appeal was unsuccessful. The Lodge will now file suit on behalf of those affected members who request to be added to the class. While the Lodge currently has the names of several members, we want to afford all members one last opportunity to be a part of this class. Please submit a request even if you have previously contacted the Lodge. Please forward this survey to anyone you believe may have had to pay the $20.00 penalty. Please note: This survey should only be completed by members who were required to pay a $20.00 penalty when filing their 2012 City of Chicago Ethics Statement of Interests. Members will ultimately have to provide proof of payment. Incomplete surveys will not be considered. |
